We have a 36 question survey, that helps people determine which personality type they are. We need a Flash version of this survey that will throw up one question at a time and then move on to the next question.

In answer to each question, the user has to indicate with a slider how true a given statement is about them, with True on the left, Not True on the right.

Once the survey is complete, the user will be asked to enter their name and e-mail address. Upon submission, two separate things should happen:

1. An e-mail, which ideally includes a dynamically generated graph image, with the survey results is sent to the user.

2. The user is opted in to our Bronto e-mail list. (We have the form logic for that.)

3. A nice bonus (but not necessary) would be a way to update the survey through a user interface in the browser.
